Nexmed is a proof-of-concept patient-owned health record (PHR) ecosystem built on the HL7 FHIR R4 standard. It consists of two interconnected products: Nexmed Passport (patient portal) and Nexmed CRM (clinic tenants), communicating through FHIR transaction bundles over OAuth2-authenticated REST.
The goal is to demonstrate that patients can own, control, and share their complete medical history across multiple healthcare providers — without relying on any single institution or proprietary format.
Patient portal
Where patients create profiles, manage records, book appointments and control record visibility across the ecosystem.
Clinic tenants
Independent clinic environments with isolated databases, connected to Nexmed Passport via bidirectional FHIR communication.
Problema abordată
În majoritatea sistemelor medicale, dosarele pacienților sunt deținute și izolate de instituții individuale. Un pacient care vizitează mai multe clinici nu are o vedere unificată a propriului istoric, iar clinicile nu au un mod standard de a schimba date între ele.
This leads to repeated diagnostics, missed context during treatment, and patients having no practical control over their own health data. Nexmed explores whether a FHIR-compliant, patient-centric architecture can address these problems in a functional proof-of-concept.
Abordare tehnică
HL7 FHIR R4
All health data is modelled and exchanged as FHIR R4 resources — Patient, Practitioner, Appointment, Encounter, Observation, AllergyIntolerance, Condition and more.
Database-per-tenant
Each Nexmed CRM tenant runs on its own PostgreSQL database, provisioned automatically on registration. The central database holds the global patient registry and tenant metadata.
Per-encounter visibility
Patients control visibility at the encounter level via a public/private toggle in their Nexmed Passport. Only public encounters are shared when a clinic requests a patient's history.
Construit cu
Scope and limitations. Nexmed is an academic proof-of-concept, not a production medical system. The consent model is a per-encounter public/private toggle — granular per-clinic access control is outside the current scope. No clinical compliance certification is claimed.
